RELEASE STATEMENT


The names printed and signed above agree to hold harmless A-1 Amusement Masters Productions, Inc., its’ employees, agents and representatives for any and all injuries and or property damage sustained while participating in any of the games or activities provided by A-1 Amusement Masters Productions, Inc. I also certify that I have no physical condition or injury that would make participation in said event a danger to my mental or physical health. I understand that these games and activities are sports, very physical in nature and that injury could result. I have read this release and understand it. I hereby waive any and all rights to claims that I may have for any type of injuries that may occur due to my participation. I am participating of my own free will and I fully accept any consequences of my voluntary actions.


PHOTO RELEASE


I hereby consent to my (my child’s/children’s) photograph being taken for any lawful purpose for CURE Childhood Cancer, Inc.

I hereby irrevocably authorize CURE, its divisions, successors and assigns, and those acting with its permission and upon its authority, to copyright use and publish, for art, advertising, medical, trade, commercial and any other lawful purpose, any in whole or in part, including any composite photographs or photographs distorted in character or in form, whether in conjunction with my name, a fictitious name or no name, or any reproduction or variation thereof by whatever medium made.

I hereby waive any right which I may have to inspect or approve any such photograph or likeness or the use to which it is put, and acknowledge that except for the consideration recited I shall receive no payment or other remuneration for use of any such photograph or likeness.

I hereby release CURE Childhood Cancer, its divisions, successors and assigns, and those acting with it permission and upon its authority, of and from every liability, granted above or any blurring, distortion, alteration, optical illusion or use in taking or publication of such photograph or other likeness unless it can be shown that publication thereof was for the purpose of subjecting me to conspicuous ridicule, scandal, or indignity.
